Why use a credit card payment form?

Most small businesses don't need a full Stripe checkout — they need a clean way to take a card for an invoice, a deposit, or a one-off charge. The accountant emails a form. The client fills it in. The card gets keyed into the merchant terminal later that day. A credit card payment form replaces the back-and-forth of "can you email me your card details" with something defensible. You get a timestamped record, a signed authorization, and a single document your bookkeeper can file against the invoice. No screenshots of card numbers in email threads, no card details scribbled on a napkin at the front desk. It works for recurring charges too. A property manager collecting monthly rent, a gym billing personal-training packages, a dentist taking a deposit for a procedure — each one needs the cardholder to acknowledge the amount, the frequency, and the terms. One form, one signature, one record per customer.

Key features of this credit card payment form template

Everything you need to launch this form in minutes, share it with your team, and collect submissions reliably.

1

Signed authorization on every record

Every submission captures a typed or drawn signature alongside the amount and date, so each charge has a defensible paper trail your bookkeeper can file against the invoice.

2

Charge-type selector

One-time, recurring monthly, or deposit — the form adapts the wording of the authorization so the cardholder knows exactly what they're agreeing to before they sign.

3

Webhook routing to your finance stack

Push each submission to QuickBooks, Xero, or your internal billing tool through a webhook. Card details land where your team already keys them, not in a separate inbox.

4

Slack alerts for the finance channel

Get a ping in #billing the moment a customer submits — useful when you need to run the card before a service is delivered or a shipment goes out.

5

File upload for matching invoices

Customers can attach the invoice PDF they're paying against, so reconciliation is a 10-second match instead of a guessing game across three emails.

6

Works on any device

Customers complete it on a phone in a waiting room or a desktop at home. The signature pad and number pads adapt to the device so nobody is pinch-zooming to enter a CVV.

Perfect for

Small business owners taking card-not-present payments, accountants and bookkeepers collecting against invoices, property managers running monthly rent, medical and dental offices taking deposits, gyms and studios billing memberships, freelancers invoicing retainer clients, and event organizers collecting registration fees offline.

Ways to use this template

Phone-order intake

A bakery taking a custom-cake order over the phone sends the customer a payment link mid-call. The customer fills in the card details and signs before hanging up, and the baker has a deposit recorded against the order.

Recurring rent collection

A property manager with 40 tenants uses the form for monthly rent on cards. Each tenant signs a recurring authorization once, and the manager re-runs the card each month with the signed record on file.

Invoice payments for small B2B

A freelance designer attaches the invoice PDF in the form's file upload, the client pays against it, and the bookkeeper reconciles invoice number to submission in one click.

Medical and dental deposits

A clinic collects a non-refundable deposit when a patient books a procedure. The signed authorization protects the practice if the patient disputes the charge later.

Event registration overflow

An event organizer running an in-person workshop takes walk-up payments on a tablet at the door. The form runs on the staff member's phone and the card gets keyed into the terminal at the end of the day.

Membership and class packages

A pilates studio sells 10-class packs and monthly memberships through the same form. The charge-type selector flips the wording from one-time to recurring so the member sees what they're signing up for.

Workflow integrations

Connect this form to your existing tools with no middleware. Every submission triggers your workflow automatically.

Why choose Flexform?

Most payment-form builders give you a fixed template and a take-it-or-leave-it field list. FlexForm doesn't. If your accountant wants the authorization wording to read a specific way, or you need to capture a tax ID for a B2B charge, you describe the change in plain English and the form updates. No HTML edits, no waiting on a developer. Submissions don't sit in a dashboard either. Route them to Google Sheets for the bookkeeper, drop a notification into Slack so the finance lead sees every charge as it comes in, or fire a webhook into Xero or QuickBooks the moment the cardholder signs. The integrations are native, not glued on. If you run a payments product or a customer portal, you can embed the same form inside your Android and iOS apps through our Kotlin Multiplatform mobile SDK — the rendering and validation match the web version exactly. For everyone else, the shareable link works without an account on the customer's side, and the free plan covers most small operators forever. Upgrade only when you outgrow it.

Frequently asked questions

Is this form PCI compliant?

The form itself collects the card details the same way a paper authorization would — it's designed for card-not-present scenarios where you key the card into a PCI-compliant terminal or processor afterward. If you need full PCI Level 1 tokenization with no card data touching your systems, you'll want to pair this with a Stripe or Square integration rather than use it as a standalone processor.

Can the customer pay directly through the form?

Not by default — this template captures the authorization and card details for you to run through your existing merchant terminal or gateway. If you want live charging, connect a webhook to Stripe or a payment processor and the submission can trigger the actual charge automatically.

How do I change the authorization wording?

Open the form in the editor and tell the AI what to change — for example, "rewrite the authorization checkbox to mention our 30-day refund policy" or "add language for a recurring monthly charge." The wording updates without you touching any code.

Where do submissions go?

They land in your FlexForm dashboard immediately and can be routed wherever your finance workflow lives. Most operators send them to Google Sheets for the bookkeeper plus a Slack alert in the billing channel, but you can also fire a webhook into QuickBooks, Xero, or your own billing system.

Can I embed this in my own app?

Yes. The form runs through our Kotlin Multiplatform mobile SDK on Android and iOS, so if you're building a customer portal or a field-service app, the payment form looks and behaves identically across web, Android, and iOS without you maintaining three separate implementations.

What happens if a customer disputes the charge later?

You have a timestamped submission with the cardholder's signature, billing address, and the exact authorization text they agreed to. That record is what most processors ask for during a chargeback dispute, and it's far stronger evidence than an email exchange.

Is there a free plan?

Yes — FlexForm has a free forever plan that covers most small operators. You only pay when you need higher submission volume, advanced integrations, or team features.

Can I take recurring authorizations?

The Charge Type field lets the cardholder pick between one-time, recurring monthly, and deposit, and the authorization checkbox text adapts to match. You keep the signed record and re-run the card on your processor at the agreed cadence.
